Left atrial tamponade.

چکیده
Post-operative cardiac tamponade is a surgical enmergency. The outcome depends on early diagnosis. The clinical picture of tamponade due to inflammatory pericardial effusion has been described by Stewart, Crane, and Deitrick (1938), Besterman and Thomas (1953), and Burch and Phillips (1962). Rapid accumulation of fluid or clot in the pericardium, as in post-operative tamponade, produces its own diagnostic problems, signs, and differential diagnosis. Unlike fluid blood, clot can produce local compression of the heart. We believe that some cases showing atypical features may be due to compression of the left atrium by clot behind the heart. Two cases are described, and the differential diagnosis of postoperative tamponade is discussed.

Cardiac tamponade in left ventricular dysfunction.
Echocardiographic and hemodynamic data were measured in closed-chest dogs during graded cardiac tamponade (pericardial pressure 5, 10, and 15 mm Hg) before and after production of diffuse ischemic left ventricular dysfunction. Left ventricular dysfunction was produced by intracoronary injection of nonradioactive microspheres (54 +/- 3.9 mm diameter). Changes in left atrial pressure with cardiac...
متن کاملEchocardiographic Sign of Regional Cardiac Tamponade
Background. Cardiac tamponade after cardiac surgical procedures is often associated with hemodynamically significant localized pericardial effusions. The localized collection of pericardial effusion in the postoperative period and the atypical presentation of cardiac tamponade limit the use of conventional clinical and echocardiographic signs usually seen with a circumferential pericardial effu...

متن کاملAtrial transmural pressures during experimental pericardial tamponade.
To elucidate the mechanism responsible for the occurrence of negative transmural atrial pressures in pericardial tamponade, saline or air was progressively injected into the pericardial sac. In 14 anesthetized, open-chest dogs, left atrial (LA), left ventricular (LV), and pericardial pressures (PP) were measured with catheter tip manometers. In beating and fibrillated hearts, measurements were ...
متن کاملLeft atrial laceration with epicardial ligation device.
Many new devices and techniques are being developed to attempt a reduction in embolic stroke risk for patients with atrial fibrillation who are either unable or unwilling to maintain long-term anticoagulation. One of these new devices (LARIAT®, SentreHEART Inc., Redwood City, California, USA) employs delivery of an epicardial suture to ligate the left atrial appendage after percutaneous pericar...
